New Program Designed to Reduce Hospital Readmissions and Make Home Care Transition Easier for Patients

MONTGOMERY, AL — Turenne PharMedCo and Healthcom are announcing a new care transition initiative that provides patients a direct mobile link to their healthcare nurse. CareLink Connect® is designed to help home health agencies and skilled nursing facilities improve care transition services, reduce unnecessary readmissions and improve patient outcomes by making communications between health care provider and patient easier.
This initiative is designed for patients who are transitioning from acute care settings (such as a hospital or skilled nursing facility) to their home. Each patient will receive a free mobile nurse help button, which can be used to connect them immediately to their homecare nurse, family caregiver or medical authorities 24-hours a day, 7-days a week. This allows quick access to healthcare professionals or responsible caregivers when important healthcare decisions are being made.
The difference with this mobile connection device is that the trained call center staff member can directly access the patient’s nurse, family caregiver or medical authorities, whenever the patient requests it. This is in addition to having access to family, friends and emergency services, which medical alert services typically provide.
“When a patient is transitioned from full time medical care back to their home, it can be a big and sometimes daunting adjustment. Increased communication during this transition time is critical,” said Aaron Kirk, President of Healthcom. “Patients are encouraged to use their button for any questions or assistance they might need to help ensure a smooth transition.”
Help may include setting follow-up physician appointments or addressing more health concerns that might require immediate attention. Successful and easy communication can also reduce the chances for unnecessary readmission.

The Centers for Medicare and Medicaid Services (CMS) announced that it will penalize over 2,600 hospitals an estimated $420M due to high readmission rates as defined under the Affordable Care Act for FY 2016. This creates a new opportunity as hospitals look to care transition partners to decrease avoidable readmissions and the penalties associated with them.
