Obituary:  Mrs. Anne Kozak Smith

 

Mrs. Anne Kozak Smith of Slocomb passed away Friday, January 16, 2015 at Noland Hospital (SAMC).  She was 88.

 

Memorialization was by cremation.

 

Anne was born January 9, 1927 in Czechoslovakia to the late Joseph and Sophia Tusakova Kozack.

 

In addition to her parents, her husband, Charles C Smith Sr. preceded her in death.

 

She is survived by her son, Charles C. Smith Jr. as well as other relatives and many friends.
